CREDIT REPAIR CONTRACT, ELECTRONIC SIGNATURE & LIMITED POWER OF ATTORNEY
 
You have contracted HOME SCORE SOLUTIONS (“HSS)to provide credit bureau dispute management services and credit score consulting.
This is a month-to-month agreement. HSS provides service on an incremental basis charging only after the initial review/setup and at the end of each month of service. You may cancel at any time with no further obligation. HSS does not charge termination fees or penalties.
In consideration of HSS's services you agree to the following:

BASIC PROGRAM $49.00

1) You agree to provide HSS with a 3-bureau credit report to start, and a new report from time-to-time, to update your progress, upon request.
2) You agree to cooperate with HSS in the review of your credit history and the identification of inaccurate or questionable items on your credit report.
3) You agree to pay HSS a review/setup fee of $49.00
4) You agree to pay HSS a monthly fee of $99 at the end of each month of service.
5) You agree to grant HSS permission to withdraw the specified review/setup fee and monthly fees from your credit card or bank account.
6) You agree to provide HSS with legible copies of all correspondence you receive from the credit bureaus during the span of the contract.
7) You agree to promptly notify HSS of any change of your name, address, or contact information.
For individuals, the review/setup fee is $49 and the monthly fee is $99.00.
Your initial review and setup includes an analysis of your credit report, credit score recommendations appropriate to your needs, and preparation for the dispute process. For the duration of the contract HSS will systematically prepare and mail dispute letters to the credit bureaus. All letters are sent on your behalf and in your name.
 
Our 90 Day Guarantee
If you are not satisfied for any reason, just tell us why and we will refund your last month’s payment in full. This guarantee is effective for 90 days from date of enrollment and applicable to current, active clients who have met their obligations under the terms of our contract. This guarantee is not meant to imply a guarantee of a specific outcome.
 
ELECTRONIC SIGNATURE
By checking the box on our application form that states: "By checking this box and providing my Social Security Number I certify that I am at least 19 years of age, acknowledge that I have read the legal disclosures, and give my electronic signature and limited power of attorney for HSS to perform work on my behalf" you are representing and agreeing that you accept all terms and conditions as if you had physically provided your signature.

LIMITED POWER OF ATTORNEY

This is a Limited Power of Attorney providing HSS with permission to represent you as your disclosed or undisclosed agent when writing, signing, and sending letters to the credit bureaus and other parties as deemed appropriate and in accordance with the purposes described in this Agreement.

You may cancel your electronic authorization and your Limited Power of Attorney at any time by sending HSS an e-mail stating that you retract your electronic authorization. Without this electronic authorization and Limited Power of Attorney, HSS is unable to represent you, and canceling it will terminate this Agreement.

OUR CANCELLATION POLICY

Cancellation Policy: Services are provided on a month-to-month basis and clients may cancel their services at any time at least (10) ten days in advance of any upcoming recurring payment. When cancelled according to these terms, the upcoming recurring payment will constitute the final payment due for services rendered.  All services are billed in arrears.

RIGHT OF CANCELLATION

You may cancel this Agreement without penalty or obligation at any time before midnight on the 3rd business day following the date on which you signed this agreement. See the attached Notice of Cancellation form for an explanation of this right.

Non-Profit Credit Counseling Services Notification Consumers should be aware that non-profit credit counseling services are available which provide a range of financial and money management services. Additional information is available at the Federal Trade Commission website.
